What We’re Looking For…
Currently, we are seeking a full-time Care Navigator to join our team! The Care Navigator implements the dementia care management program, including the CMS GUIDE (Guiding an Improved Dementia Experience) Model at Shell Point Retirement Community, supporting residents with dementia and their caregivers. Based in the primary care clinic, this role collaborates with the interdisciplinary healthcare team to deliver comprehensive, person-centered care. Key duties include conducting assessments, providing education, and coordinating services. The Care Navigator also ensures interdisciplinary collaboration to enhance outcomes and achieve the program’s goals to improve quality of life for people living with dementia, reduce strain on their caregivers, and enable people living with dementia to remain in their homes. The ideal candidate will have a Bachelor’s degree in social work, nursing, or related field, a minimum of 5 years of experience in social work, care coordination, or case management in a healthcare or community-based setting working with individuals living with dementia and caregivers. Experience in dementia care is required. Certification such as CDP or other relevant training/certificate is preferred.
