Job Description

We have a challenging opportunity providing comprehensive geriatric case management for independent and assisted living residents within a large continuing care retirement community (CCRC). This position is responsible for helping residents and their families manage difficult life situations by conducting assessments of resident’s needs and level of functioning, collaborating with family and medical providers to develop a plan that addresses needs and identifies strengths and resources as well as coordinating services. This will include assessing a resident’s ability to live in their current setting and if necessary, encourage and facilitate a move to a higher level of care so that dignity, safety and optimal independence can be maintained. A social services background is strongly preferred; however, we may be interested in individuals with a related background who have the proven ability to empathize and demonstrate compassion with aging individuals. A Bachelor’s degree is required.
